Hand off all of it or just the parts that are breaking. Say which in the form so the proposals match.
Charges entered, claims checked against payer edits, and sent daily so clean claims go out the first time.
Every denial worked, appealed, or corrected, with the root cause tracked so the same denial stops repeating.
Old balances called on and worked by payer and by age until they are paid, adjusted, or written off on purpose.
Certified coders assign CPT, ICD-10, and modifiers from your notes, or audit the codes your providers pick.
New providers enrolled with payers, CAQH kept current, and revalidations handled before they lapse.
Statements, online payment, payment plans, and the phone calls that come with them, handled in your name.
Eligibility checks, prior authorizations, and old AR cleanup go through the same request. If you only need one piece, say so.
There is no right answer for every practice. The usual tradeoffs:
Fertility benefits are a recruiting tool at Bay Area tech companies, and they have changed how OB/GYN practices bill. A patient may have coverage for evaluation, medication, monitoring and procedures, or only some of them, through a separate vendor that administers fertility benefits apart from the main health plan. The OB/GYN's office has to bill the right vendor for the right piece, and the plan will not tell the office where to send it. Diagnosis codes matter here. An infertility diagnosis may unlock coverage that a general gynecologic diagnosis would not, and coding it correctly requires documentation of the evaluation. Ultrasound monitoring during cycles adds many small claims, each needing authorization.
For regular obstetric care, San Jose practices work with Stanford, El Camino Health, Sutter and county hospital deliveries, with Kaiser handling its own members. Delegated groups pay many global packages, and a practice that works across groups has to know each one's rules. Gynecologic surgery moves through ambulatory centers. When you evaluate a billing company, ask for a walkthrough of one infertility patient's claim path from consult to final payment. If the explanation is vague, they have not done it.
